چکیده

hinotecan (7.ethyl-1O.[4-(1.piperidino)-1-piperidlnolcarbonyloxy. camptothecin (CPT-11) Is hydrolyzed by the enzyme carboxyl esterase to 7-ethyl-lO-hydroxycamptothecin (SN-38), which further undergoes glucu ronic acid conjugation to form the corresponding SN.38 glucuronide (SN-38G). SN-38 is believed to be the cause oftreatment-related diarrhea, a dose-limiting tOxiCityof CPT-1l observed in phase I clinical trials This study investigated the effect of glucuronidation on the concentrations of SN-38 following CPT-11 Infusion In 21 patIents undergoing a phase I triaL To assess the relationship between gastrointestinal tOXiCItYand pharma cokinetics of CFr-11 and its metabolites, we defined a “biliary index― of SN-38 which was the product ofthe relative area ratio ofSN-38 to SN-38G and the total CPT-11 area under the plasma concentration-time curve. Nine patients with grade 3-4 diarrhea had higher biliary Indexes than 12 patients with grade 0—2diarrhea (median 2228 versus 5499, P 0.0004). The relatively higher index values, suggestive of higher billary concentra tions of SN-38, were possibly due to low glucuronidation rate& Hence, modulation of glucuronidatlon may be effective in Increasing the thera peutic index of CPT-11.

Genetic predisposition to the metabolism of irinotecan (CPT-11). Role of uridine diphosphate glucuronosyltransferase isoform 1A1 in the glucuronidation of its active metabolite (SN-38) in human liver microsomes.

Irinotecan (CPT-11) is a promising antitumor agent, recently approved for use in patients with metastatic colorectal cancer. Its active metabolite, SN-38, is glucuronidated by hepatic uridine diphosphate glucuronosyltransferases (UGTs). The major dose-limiting toxicity of irinotecan therapy is diarrhea, which is believed to be secondary to the biliary excretion of SN-38, the extent of which is ...

Glucuronidation of 7-ethyl-10-hydroxycamptothecin (SN-38), an active metabolite of irinotecan (CPT-11), by human UGT1A1 variants, G71R, P229Q, and Y486D.

7-Ethyl-10-hydroxycamptothecin (SN-38), an active metabolite of antitumor agent irinotecan (CPT-11), is conjugated and detoxified to SN-38-glucuronide by UDP-glucuronosyltransferase (UGT) 1A1. Genetic polymorphisms in UGT1A1 are thought to contribute to severe diarrhea and/or leukopenia caused by CPT-11. In this regard, it has been reported that polymorphisms in the promoter region could affect...

Insights into the pharmacokinetics and pharmacodynamics of irinotecan.

Irinotecan is a relatively new anticancer agent of interest for both its clinical activity and its complex clinical pharmacology. It is a prodrug, requiring activation by carboxylesterases to SN-38, an inhibitor of topoisomerase I. Recent studies suggest that human carboxylesterase-2 is the primary carboxylesterase involved in the hydrolysis at pharmacological concentrations (1). Irinotecan is ...
